Dozo Dog can only provide service if all of the following vaccinations have been uploaded and validated by picture, screenshot, or PDF file, to your profile which is accessible through our customer portal.  

Required Vaccinations:

Signed Rabies Certificate
   Signed Rabies certificate must include:

DAPP - Distemper, Adenovirus, Parainfluenza, Parvovirus
 
Bordetella

  • Owners information

  • Dogs information

  • Manufacturer of vaccine

  • ​Serial/Lot number

  • Serial/Lot number expiration date

  • Date vaccination was given

  • Date vaccination expires
